How to Send Your Referral
Many clinics choose to send their referrals directly to us through their EMR system. You can also download and fill out the referral form below and fax it to the preferred NWOS location listed. We will then contact your patient directly to schedule an appointment within 48 hours.

Urgent Appointments
For injuries, fractures, new back/neck pain, or other orthopedic concerns that require immediate treatment, please refer your patients to one of our three Express Walk-in Clinics - North Division, South Hill, or Spokane Valley.
